Enhancing Medication Management in Hemodialysis Patients - Exploring the Impact of Patient-Centered Pharmacist Care and Motivational Interviewing

This study evaluates the effectiveness of pharmacist-led interventions in managing medication-related problems (MRPs) for hemodialysis (HD) patients using Drug Therapy Optimization (DTO) and Motivational Interviewing (MI). Over 12 months, pharmacists identified and addressed MRPs through patient interviews and medical reviews, with a significant reduction in patient-related MRPs. The study shows that combining DTO and MI can improve medication adherence and reduce drug-related issues in HD patients. Applications include optimizing pharmaceutical care and improving patient outcomes for those with End Stage Renal Disease (ESRD).
